1995 Ford Taurus oil leak

Our car steadily leaks oil, and needs to be virtually re-filled every 300 -400 kms. Also, when in the lower gears and when changing gears it produces alot of smoke from the engine, I thought that this could be oil leaking onto a hot element and burning off.

Well I guess the first step would be to get it up on a lift, or the very least car ramps to have a look-see.
