A leaking head gasket or a cracked head. This usually occurs when the car overheats.
This needs to be repaired ASAP, because if it is actively leaking in the cylinder, you take a chance that you will "hydraulic" the engine. Which means that coolant leaking in the cylinder will accumulate to the point where the piston will try to compress it (liquids are not compressable) and you will bend a rod or break the crank.
