Update: I took my truck in. They found the left side front and back had separated belts. I bought two new tires put them on the front. Balanced all. And severe shaking is gone. Great diagnosis guys. Much appreciated in your help. Now I am going to return all the parts I bought. I will get two more new tires soon and hopefully it did not wear out my front end too bad.

That would do it. Probably hit a nice pot hole or something in the road and popped the belts, that in turn shook the weights off and made it worse.
