1999 Honda Accord Jerks into gear when accelerating from a

From a complete stop, it will rev and then jerk into gear when I accelerate. It goes through this for the first two gears then shifts fine. When I come to a stop it will jerk to the low gear about 2 or 3 seconds after I've stopped. It only does this after it has warmed up (after about 45 min drive time). I have tried to accelerate very slowly to give it time to shift and it doesn't help. The trans. Fluid is good, was changed within last 6 mo.
