My front wheel bearings keep coming loose. I take the assembly apart, retighten the nut, and install all the parts back properly. Then as soon as I go down the road again I can hear grinding again. When I get back, the nut has backed off somehow even with the key inserted.

How does this keep happening?

Hey,
Sounds like the "locking pin" on the inner nut has broken. I recommend you replace the inner nut and be sure to torque this one to spec!
