Coolant Leak

Good Morning. I have a 1998 Dodge Neon 2.0 SOHC. I have a coolant leak that I am not sure how to repair, mostly what parts I need though. The leak has become somewhat bad because I have to add fluid every 2-3 days. The leak is at the connection between the Head and the main hose where the Radiator cap is. Can anyone tell me what I need to do so that I can solve this problem and not have my car overheat anymore. BTW, I'm not sure if it matters but I had that problem with the cooling fans not coming on for several months before this, but they are fixed now.

Sounds like a loose hose clamp or bad radiator cap.

Ok, let me be a bit more clear here. I see where the leak is, it is coming from the conection of the Main Hose with the Radiator cap, to the Engine, where the hose does not clamp in but is bolted in. I have checked both the radiator cap and all the clamps and they have replaced all that are bad. Can anyone tell me how and what to replace to solve this problem.

Drain coolant, unbolt rad cap/thermostat housing from the head, clean surfaces, install new gasket and reassemble then refill with good coolant
