Can a head gasket be bad if the car is not running hot?

Yes. It can be intermittent depending on driving conditions and ambient temperature. It will occasionally overheat but other times drive find without an issue. Look for colored spark plugs with a green hue. Watch for coolant being pushed into the cooolant overflow and then the car overheats. The exhaust will be white and smell sweet. Finally coolant mixing with oil or oil mixing with coolant is a dead giveaway.
