Engine gets really jerky and will hardly accelerate

I have a 1987 mk4 ford escort 1.4gl. My car will usually start and idle fine, but when I drive it usually gets really jerky and will hardly accelerate. Also when I slow down to a stop it stalls. Then it idles very rough with the revs going up/down then stop.

Could also be weak fuel pressure.
