I had a mechanic look at my car and he said it needed a new head gasket and that was it. He did the repairs. A week later the car started making a knocking noise. The same mechanic is now telling me there is metal in oil and it needs a new rod. Is this something he may have caused or should have noticed when putting in the new head gasket?

Good evening.

When a head gasket fails, coolant gets into the oil. When it does, it damages the main and rod bearings. The clearance increases and the result is the noise.

I never do a head gasket without replacing the main and rod bearings just for this reason. He should have known this if he is a technician. He did not cause the issue as it is a result of progressive damage from the head gasket failure.

You will now need an engine overhaul to repair this issue.
