Losing power going uphill?

My car has been doing this for a while now, every time I am driving and I go up a hill it loses power, and I lose speed unless I hold the gas pedal all the way down pretty much. Now lately it's been kind of cutting out and/or missing, if I'm doing 60 it'll be driving nice and then it seems like for a split second it loses power or something. I've changed the spark plugs and cleaned the mass airflow sensor and the throttle positioning sensor and I'm not sure what it could be. Let me know your thoughts. Thanks.

It sounds like the catalytic converter is getting clogged, do you have a check engine light on? If so, let run the codes to see which ones are stored.
