heater/AC blower will come on sometimes when the car is started and may come on for several starts, then go off and stay off for several starts and then come back on. When the blower is working all controls work, (fan speed, temperature control, etc.). I have checked all fuses and fan motor.

There is a fan speed controller that sits just outside of the heater box that sounds like it has gone bad but to know for sure test the power wire headed to the blower fan when it is not working. If you do not have power going to the fan then suspect the module if there is power then the blower motor has gone bad. Here are some guides and diagrams that will help you get started.
