When I start my car thick white smoke comes out the tailpipe and my car shakes. When it is in park it revs up and down on its own.

Guess what? It is time to have a head gasket replaced. Prepare to either spend some cash or save a bundle by doing it yourself. The head itself should be taken to a shop, usually a lot of auto parts places have a shop to do this, and have the head checked for cracking and warping. Don't know what else to say, but the decision is yours on whether to fix or not. I would definitely shop around at GOOD mechanics with experience on Honda and get estimates.
