Based on all this info, I suspect we have a vacuum leak. However, we need to check all the basics and find out what is going on.
Let's start with checking for a leak and then fuel pressure. Then we can move onto greater things.

Once we rule these out then we need to check for internal vacuum issues by putting a gauge on the engine and see if the needle is steady or it bounces around.
I am attaching a chart below that will help with this but let's start with this info and see what direction we need to go.
