why does the surpintine belt keep falling off the pulleys

The tensioner pullet is worn, the whole pulley system is not aligned correctly, or you have the wrong belt. In that order.
If your not sure, eyeball the pulleys for alignment, then put a Mopar tensioner, and new belt on. Anything else would be unusual materials constantly getting in the system.
