Idle speed normal when started hot, also normal when started cold but does not adjust as engine warms up

98 Civic EX 4-door Automatic. 258K miles.

When cold engine is started, idle goes to 1800 RPM and in a few seconds settles at about 1400 RPM which is OK. But when the engine warms up the idle remains at 1400 which is way too high. It stays at 1400 no matter how far I drive, if I shift to neutral. With the transmission in Drive, stopped at an intersection, RPMs drop to 1000-1100 due to resistance from the transmission, but it is pulling pulling pulling, and if I shift back to Neutral it immediately jumps up to 1400 again.

HOWEVER, if I shut off the engine and restart it hot (anywhere from 2 seconds to perhaps 30 minutes after shutoff), the idle goes first to about 1200 then quickly settles to 700-800 RPM, right where it should be, and stays good no matter how far I drive. After warmup, often I don't even crank the engine to restart, I just turn off the key and turn it back on while driving, so the engine restarts like with an old-fashioned push start, then when I stop at the next intersection the idle is fine.

In all cases the idle is stable -- that is, it doesn't fluctuate.

IACV was thoroughly cleaned, and then replaced with a new authentic Honda IACV. No change.

So what's going on and how do I fix it?

Cannot clean the iac valve. I would replace it. It is the motor portion that fails.

Look for vacuum leaks around the motor as that is also common for high idles.

Make sure the throttle plate is clean and returning to the closed position at idle

Thanks for the suggestions, but remember that idle speed is fine after a HOT-engine start, it is only too high when the engine starts cold and warms up from cold to hot. If IAC is bad or there is a vacuum leak or open throttle plate, why would shutting off a hot engine and immediately restarting it cure those problems?

OK, figured it out. The throttle position sensor was mis-adjusted so the throttle appeared to be open even when fully closed. Since the computer never saw the throttle as closed, it did not see any need to adjust the idle speed via the IACV.
