My 95 Monte Carlo had a spark plug blow out of the engine. What would cause this?

My 1995 Monte Carlo had a spark plug blow out of the engine I replace all spark plugs and wires with a new ones. Now it runs like crap and has a hollow sound. What could cause this?

Did the threaded part stay in the motor?
How olad were the plugs?
The only thing I can think of is that the ceramic and casing failed and the compression form the cylinder blew it out.
