I suspect it is a failed head gasket. The easiest thing to do is check engine compression to see if it is lower in that cylinder. Here is a link that shows how to check engine compression. Also, pay attention to see if there is any white smoke from the exhaust and confirm the motor oil doesn't appear to have a tan-colored sludge.

Here is a link that explains how to test for a bad head gasket which you may want to do first simply much of it is visual and won't require the compression test until certain criteria have been met.

Keep in mind, the idea that the coolant level has been an issue doesn't only indicate a leak. It could be getting burned in the combustion chamber, mixed into the engine oil, or leaking.
If you can take a picture of the crust on the manifold. I realize you are having trouble getting a good pic, but as long as I can see it, I should be able to tell if it's coolant.
