Could be a bad solenoid in the trans or a bad sensor. Your best bet would be to have the PCM scanned to see if the trans has recorded any trouble codes. If it is a controllable item, the PCM will record a fault, if it is a mechanical issue in the trans, it may not record a fault code, but this is the best place to start.
