The usual causes are: head gasket leak, warped/cracked head, intake gasket leak, warped/cracked intake. More rarely, the engine block's water jacket may be cracked or rusted through, in which case the entire engine block must be replaced.
Have the problem diagnosed immediately, the longer the problem exists, the more damage is done and the more it will cost to fix.
