Hi and sorry for the news. However, it sounds like that is what is happening. Don't laugh, but the easiest way to determine if the clutch is bad is a simple trick I was taught years ago. In an area that it is safe, place the transmission in fifth gear. Give the engine some gas and let out the clutch. If the engine doesn't stall, replace the clutch. Sounds crazy, but a good clutch will cause the engine to stall.
As far as price, I don't know where you live and what the going rate is for labor. However, I can tell you the book calls for hours in labor for the job. So, multiply shop rate by that number to determine approximately what the labor should cost.
Next, in addition to replacing the clutch, the pressure plate and throw out bearing should be done at the same time. There should be no additional labor because these parts come out with the clutch disc. What to do is call local parts stores and tell them you need a price on a clutch set for your vehicle and confirm that the price they quote you includes the aforementioned parts. Again, I don't know where you are located, so prices will change. If you were here (Pittsburgh, PA) you could expect to pay under $150.00 dollars. I just looked one up on line and the entire set (aftermarket from a nationally recognized parts store) started at $102.00 for everything.
