1997 Ford Ranger Acceleration and deceleration jerking

Whenever I drive anywhere and press the gas to accelerate, the truck clunks or jerks. The same thing happens when I let off the gas, except it will lag for a second or two when the gas is released. Then all of a sudden it feels like the gas is completely cut off from the engine and it will jerk violently. I've replaced the throttle position sensor thinking that would be the cause but it wasn't. I'm hoping it's not something more serious but it probably is considering the age and mileage of the truck.

Did it just happen all of a sudden? Drive with vehicle and thow in park do you hear a groan sound?

When passing someone and steeping on gas fast and off gas fast does the clunk happen?

As you say when you let off theres a lag. Can you describe that better?

The problem slowly started to occur over the past year. It didn't just happend all of a sudden. Yes it does clunk when I'm on the freeway and I step on the gas. It also does it when I let off the gas too. What I meant by lag is that there's a delay between letting off the accelerator and when the engine rpm actually drops.
