Hello,
It does sound like you might have a faulty blower switch.
The best course of action would be to inspect the switch and check to see if it has an output while it is on any other setting than the low speed.
This can be done by accessing the back of the switch and turning on the inoperative speed and see if there is power output to the wire.

If there is no power, then it is a faulty switch and will need replacement.
If it has power, then check the blower motor resistor.
