1998 Ford Escort Escort ZX2 v-tech DOHC engine is eating ti

My 1998 ford eccort zx2 is eating timing belts. Water pump was replaced and the engine had a code. Timing belt was due to be changed so proceeded to buy kit and replaced the works. Put back together and ran fine for one week and then ate the timing belt. Since then have tried twice again to get it back together each time the belt walks off the cams and frays. What could be the problem? Is there some type of guard or spacer that might have been forgotten? Do I have a bad pulley? Any help would be greatly appreciated. Thanks.

Was the tensioner. Found out new tensioner actually had humple in it that caused the belt to walk off and eat the belt. Manufacturer defect that drove us crazy. Car fixed and running great now, thanks!
