Radiator coolant fan not working properly

I have a 2001 dodge stratus r/t 3.0 l v6 with 95,000 miles. My radiator cooling fan does not turn on often enough or stay on long enough, causing my car to run hot off and on throughout the day. I am considering changing the relay but was wondering if you know what might be causing this before I do anything with it. Any information you can give me would be greatly appreciated.

At what temp does it come on, and what temp does it go off?

My temp gauge does not show the temp, but when I am in stop andgo traffic it will turn on for about five seconds and not turn back on for a while.
