Van shakes while accelerating

My van shakes when I accelerate at speeds 45 MPH+. If I'm going say 60 MPH and let off of the accelerator, the shaking stops. If I accelerate at speeds below 45 MPH, the shaking is not noticeable. So in order for it to shake, you have to be both going over 45MPH and accelerating.

Any ideas?

Check tires (Tread separation) and have them balanced, check the cv's (Axles) and motor mounts, if the mounts are bad the engine can shift causing bad angles on the cv's which can cause vibrations. Does it do it if you manually hold the trans in different gears?

If the tires are out of balance, wouldn't it shake when I'm not accelerating as well? Just wondered.
