When I'm going down the road at any speed usually, I have on my cruise control and if I go to accelerate hard, say to pass somebody on the interstate, there's a real hard jerk in the rear of the car every time. It sounds just like it does if you're pulling a trailer and it gets a little slack in it and you move forward fast enough, and it just falls back into place.

It sounds like you have a rear CV axle that needs to be replaced. Can you please shoot a quick video with your phone so I can hear the noise? That way I can tell for sure what's going on. You can upload it here with your response. Here is what I think it could be in the images below. I have also seen the Viscous Coupling device go out and cause this noise as well. I would do a gear oil check in the rear differential as well, please check the center support bearing as well.
Check out the images (below). Let us know what happens and please upload pictures or videos of the problem so we can see what's going on.
