1995 Chevy Silverado cooling system leaking

I have discovered a bad cooling system leak, it is commingg from the heater hose under the hood that connects to the intake manifold, it is leaking badly at the connection going in the manifold, what is the procedure for fixing this leak? Thank you

Basically, all you need to do is remove the clamps holding the hose in place, go to the parts store, show them the old one and they will cut you a new one the same length. After you replace it, refill the coolant and run the engine and recheck coolant level.
