Very simple. I just did this today on my '99. Go to a Chevy dealer and get a recall kit: Part #15939908. It comes with two new cables and two new connecting bolts. (These are for a 2000 Chevy but it should fit yours okay.)
Use a 3/8" drill bit and drill out the existing rivets. Then use the 3/8" bit to enlarge the holes in the brackets where the rivets had been. Now remove the screws holding the mounting brackets into the tailgate. With the brackets loose, you can install the new cables with the new connecting bolts using wrenches. Then just put everything back together and you're set.
It's actually a nice install and if the cables break again, you can easily replace them with wrenches. No stupid rivets!
