Between 20MPH to 45 shakes violently only when pushing the gas! Over 45 still does it but not as violently but only does it while accelerating. I've changed all the motor mounts, what else could this be?

If you've ruled out a bent wheel or broken belt in a tire, and an engine misfire, suspect a worn inner cv joint housing, especially if the shaking is in the steering wheel and goes away when you let off the gas. It is usually more noticeable when turning and accelerating, such as when turning out of a parking lot onto an up-hill road, (more load). Holler back if you want to inspect it yourself. I've replaced about two dozen joints and shaft assemblies for this problem, and it's almost always the right one.

This article shows how to replace the entire half shaft. You can almost always find new ones today for less than $100.00, so it doesn't pay to try to repair a single CV joint:

The inner CV joint has to be disassembled to look for the cause of the vibration. Specifically, the three highly-polished rolling surfaces, (one is indicated by the blue arrow), must be inspected.
