Car shakes when braking

Car shakes when brakes are applied. Just wondering if it is the front rotors that are warped? The car does get driven a lot and the pads have always been changed when needed and they have always worn evenly. It has been doing this for almost a month now and I finally have time to look into this.

You definitely have warped rotors. Whether it is coming from the from or rear will have to be determined by where you feel it. If you feel it in the steering wheel, chances are it is the front.

Then you have a problem with the front rotors or the way they are mounted.
