1994 Stops running after braking.

When I drive around town my car is great. But recently, after I drive it at highway speeds (50-70 mph) for 20 min. Or so, it sputters, jerks, and shuts down when I'm coming to a stop (usually under 5 mph). It starts up right away, but shuts down when it is put into drive and I accelerate. If I start it, wait 10 seconds and then shift into drive and accelerate it works, but is sometimes a little slow. This happens everytime I stop thereafter, until I let it sit for a couple of hours, and then it is fine again.
Any suggestions? I have absolutely no car/mechanical knowledge, so lay terms would be great.

Ben, what I would start with is replacing the fuel filter
with driving around town the fuel demand is less than highway driving. Highway driving causes it to need more gas. If the filter is getting clogged it would cut down the amount of fuel delivered to the engine causing it to sputter or stop.
