Originally we reconditioned the cylinder head only. Customer later brought to us the entire vehicle; said he heard a bang and thinks he bent valves again. Diagnosis found we needed to take head off and check for damage. Found head gasket not torqued down, found no clamps on coolant hoses, found loose bolts on intake, exhaust, and alternator. Found spark plugs fouled with water. Initially we felt that the head gasket was blowing water past it and into the cylinders. Replaced head gasket, torqued properly, etc. Water still went into cylinder. Found vacuum hoses and water hoses were reversed. Fixed. Stated engine, had loud noise, turned out to be broken rod. Replaced. Started again, broke another rod, found water still getting into cylinders. Do you have a diagram of the coolant system so we can check coolant system routing? Also, any thoughts on what's going on?

Seems as if the block itself may have a crack/hole somewhere in the cooling jacket in the cylinder walls perhaps? Be sure timing is correct.

Are you saying that the vacuum line and coolant hoses were mistaken for eachother and installed that way? Could you just disconnect them and turn the engine over and feel what is getting vacuum for the manifold and what is pushing coolant?

The head may also be warped, and not mating well to the block.
