2004 Toyota Tacoma Power loss, hesitation, stall

At 1500-1750 rpm it seems that my tacoma stalls/power loss after about 20-30min of driving. And also at cruising speed 70 mph. The stall/power loss last about 3-5 sec. And not all the time. It also seems it happens at stop lights when I'm taking off. I had the TB cleaned along with the map sensor and it still has the problem. Could it be the TPS ? It's getting dangerousness especially at stop lights.

Has the fuel filter ever been replaced? Have you checked fuel pump pressure?

Not sure about fuel filter or pump pressure. I bought the truck about a year and a half ago. It didn't start happening until a few months back
