My exhaust emissions are "off the scale" what should I be looking at

We have no info on this car, however scan for codes, and check fuelpressure first. Then change oil and filter as well as air filter, drive about 20 miles on exspressway then get tested. If there are codes or low fuel pressure get that fixed before as well.
