My temperature guage on my 1988 honda accord doesn't rise very much. Is this something that should concern me?

It is not a problem but if the engine is running too cold, it can result in higher fuel consumption.
Check the thermostat for sticking.

I already changed the thermostat. Any other suggestions why it runs cold? I would love to get better gas mileage

Are the cooling fans working all the time?
If the thermostat is good, it would not run cold. Most people have problem with overheating insted of runing too cold.

One thing I thought about is maybe my temp gauge isn't working. Is that a possibility?

That is possible or it could be a faulty sending unit.
Without turning on the A/C, run engine until raiator fan comes on and check the temperature gauge reading to see where it is.
