It wont shift into any gears but 4th

It wont shift into any gears but 4th. Is the clutch dropped completely or is it the pressure plate, throwout bearing or what? Transmission out completely?

Yes. You need to remove the transmission and I would replace everything shown below so you don't have to go back in when the next part fails.
