Hesitates when accelerating from a stop and driving uphill

My car when at a complete stop does not want to take off or whenever going uphill, if I am under the speed of 40MPH it acts like it does not want to change gears. i don't know if it is a vacuum seal leak or if it is my transmission.

This happens when the input shaft speed sensor goes out. To be sure let's run the codes. I have included a guide to help you and the location of the sensor in the diagrams below.
