Hello,
This sounds like it could be a problem with the defrost actuator. This actuator is responsible for routing the air to defrost, vents, or floor ducts. It, like everything else, wears out from normal wear and tear over time, and when it fails, the door will be stuck routing the air to whatever position it is in when it fails. As far as the vacuum hose you are probably going to have to get it from underneath the dash and re route it through the hole in the firewall it originally went through. I have included the vacuum diagram you requested in the diagrams down below.
