If I pass it up and go to first, I can feel it shift but does not move unless I go back to D. Then truck will start moving forward. What could the problem be?

With it not shifting unless you to 1 then to drive when you go to D. I would check the linkage and cable.

I checked the linkage and cable and adjusted the shifter cable and adjusted the heat range sensor on the side of the transmission. Nothing has changed and still no reverse at all. I drove it and it does shift gears when driving and OD off light is flashing.

Here is an article to help you:

Where is the reverse sensor located. I also can not find the obd 1 diagnostic plug under the hood anywhere. Where could it be?

Look down at the transmission. You will see a cable that attaches to a lever. Look under that. You will see a black plastic part with a wire connector plugged in.
