When the engine is cold, I have issues in taking off. I have to rev the engine for sometime before it can take off, either in D or 2. When the engine is hot all the gears are working properly and this is after the engine has been running for almost half an hour.

This is a common issue when the seals and o-rings in the transmission get cold and stiff. They do not flex as well and you get internal leakage. Once the fluid warms up the seals become flexible again and the transmission starts working like it should.

The "proper" repair would be to remove the transmission and have the seals and any worn parts replaced.
The other option would be to try one of the additives. I have used Lucas brand transmission additive and it did help some. It may help in your case but it depends on how stiff the seals actually are. It would probably be worth a try and if it did not work you would not be any worse off than you are now.
