1995 Toyota Tacoma engines sputters

engine runs fine in park and neutral, but sputters and sometimes dies in drive patting the accelerator is the only way to keep it from dying. already changed the fuel somewhat improved but not completely fixed what's the next thing to do?

What is the idling speed?
Do you have performance related issues such as hesitation, misfiring etc?

Have you checked the spark plugs and related parts?
Is the Check Engine Light showing?
