1994 Toyota Pickup Jumping RPM

Needs Help. I have a 1994 Toyota Truck Extra Cab with Manual Trany. My problem is on an uphill on fourth Gear my RPM jumps from 3K RPM to 4K RPM. Does anybody thinks this is a Sliding Clutch? Or if not what causes this?

The clutch could be slipping -also have the throttle position sensor checked.
