1996 Other Toyota Models what is wrong with my transmission

I would like to know how to repair my Toyoto T-100 Truck. It pops out of 3rd and 4th gear. I would like to know if it is my transmission problem or something else.

It should be the trans, both synchroniser gears and hub are worn and the clutch shaft bearing might need to be replaced too.
