2000 Toyota 4Runner rough idling

My 4runner started to rough idle very bad when stopped or when accelerating until the RPMs reach 2000. If the RPM reaches 2000, the engine smooths out. If also doesn't rough idle unless I am pressing the gas pedal or stopped at a red light, etc. Any ideas what is wrong?

Clean out the Idle Air Control Valve and test the Throttle Position Sensor and Air Flow Meter.
