1995 Toyota 4Runner 4Runner needs help

Bought this truck off of my mechanic, 6 months later blew the headgasket ($1500 repair) a month later the car would start or not start. Not starting in the morning usually but then would run fine. Mechanic says it is the starter ($240 repair)NOW the truck starts rough, coughs kinda, and when I hit 55 (in OD or not) the engine gets really LOUD and the vibration is so intense when I accelerate the cars in my rearview are all blurry. Lower than 55 and she runs fine, tho the truck has always been loud. I am at the end of my rope here. Any advice would be welcome! Thanks!
