When I press on the gas in my van, I hear a clicking noise, I cannot tell if it is from the transmission, or from something else.
What went wrong?

There are dozens of things that cause a clicking noise, but if you only hear it during acceleration, the best suspect is an exhaust leak near the engine, typically at the exhaust manifold.
