What would cause the thermostat housing not only crack but, a chunk blew off and of course spewed anti-freeze all over. I know it overheated but what would be the root cause? Thermostat stuck, low anti-freeze?

Was any work done before this happened?
It could be from excessive pressure? If it was, that tells me the pressure cap is bad. It should release pressure at 16 lbs.

The only work done was a replacement of coil pack on cylinder #4 as well as spark plugs being replaced.

Okay, I would suspect a pressure issue resulting in the failure. I would have the pressure cap tested, the housing and the thermostat replaced.
