2000 Subaru Outback Rough idle

Whenever I brake to a stop while in drive, the engine idles EXTREMELY rough...very obviously. If I switch into neutral, it's perfectly fine. I have had it to my mechanic several times. He cleaned the injectors; still didnt' stop it. Then said there was some kind of valve (or something like that) sticking, and loosened it up; still didn't stop it. It can become EXTREMELY irritating and loud. Do you have any idea what might be causing this?

Do you have any problems accelerating? How long has the problem been going on?
