1999 Subaru Legacy CRANKSHAFT PULLEY

What would cause the crankshaft pulley bolt to back itself out and cause the pulley to come off? Is this a common occurence in a Subaru?

Not a common occurance, But there is a technical bulliten on the part itself...it's been redesigned.
