When I get up to speed to clear gears the front end will shake terribly.

So, does it seem like the engine is misfiring or are the tires shaking? Can you please shoot a quick video with your phone so I can hear the noise? That way I can tell for sure what's going on. You can upload it here with your response. It takes time to upload so please be patient. If it seems like the front end, I would raise the front of the truck up and spin the front tires to look for irregularities in the tire. Also, grasp the front tires and move them side to side to check the steering components.
